Follow-up to cmoriarty/openspec-flow#75, for openspec-flow#74, from checking the run bar on production.

Phone overflow: at 390px soundcheck#32's run pushed the page 260px sideways. The bar's grid column is minmax(0,1fr) now, so the title truncates.
Skipped phases: a run that failed in apply showed checks on ship and land, whose steps were skipped as unreachable. A phase needs a succeeded step to be done.

Both reproduced first: a browser spec with that run's title at 390px (received 260px of overflow) and a unit case for the failed run's stepper. Full lane: backend 1799, UI unit 427, browser 89 passed.

Found on production after the run bar shipped, on soundcheck#32's failed run:

- At 390px the page scrolled 260px sideways. The bar was a grid with the implicit auto column,
  whose minimum is its content's min-content width, and a truncated title's min-content is the
  whole title. The column is minmax(0,1fr) now, as the stage's already is.
- Ship and land showed checks on a run that failed in apply: the graph skips their steps as
  unreachable, and skipped counted as settled. A phase is done when its steps have settled and one
  of them succeeded; one whose steps were all skipped shows as not reached.
